Per 2025 estimates cited by eCare Behavioral Health Institute, NPD affects between 0.5% and 6.2% of the U.S. population, though up to 16% of individuals may meet the diagnostic criteria for the disorder in clinical settings. NPD primarily affects men, with Blueprints Psychiatry stating that up to 75% of diagnoses are for male patients. Regardless of gender, NPD is associated with many comorbidities, including depression, anxiety disorders, and substance use disorder, making early recognition and professional treatment important.
According to AMFM’s resource, an individual will only be diagnosed with NPD if they display five or more of the following traits: a sense of entitlement that goes beyond what is considered normal, a persistent need for admiration or attention, lack of empathy, fragile self-esteem masked by confidence, feelings of arrogance or superiority, manipulative or exploitative behavior, envy for others, obsession with power and success, lack of respect for personal boundaries, emotional volatility, a fear of loss or rejection, and chronic difficulty sustaining healthy relationships. The guide adds that individuals can have narcissistic traits without being diagnosed with NPD; the difference lies in the severity of symptoms and their impact on daily life, with narcissistic traits appearing occasionally within individuals, but NPD being a pervasive and chronic condition that impacts the affected person’s ability to function.
AMFM’s guide says that therapy, particularly cognitive behavioral therapy (CBT), can be effective at addressing distorted mindsets, improving emotional regulation, developing empathy, and building self-esteem in an individual with NPD. Beyond professional care, affected individuals can reduce the impact of their symptoms by engaging in self-reflection activities like journaling, as well as self-improvement practices.
